ONWARD REFERRAL

Onward referral to secondary care services can be made following receipt of referral from your GP at any of the following points on the pathway.

  • Directly following triage
  • Following any appropriate assessment and or treatment
  • Following any investigations

At the point of onward referral the practitioner will discuss the specialist service that they feel is the most appropriate to continue with your care.

You will then be contacted by a member of the administration team who will discuss the choice you have in selecting a hospital to arrange an appointment to see the specialist. The team will make this appointment for you directly at the hospital of your choice.

The relevant information from the SIMS service will accompany your referral.

We are able to refer on to most specialities as required for the patient’s condition the most common for patients with musculoskeletal conditions are the following-

  • Trauma and Orthopaedics
  • Rheumatology
  • Pain management

ONWARD REFERRALS TO OUTSIDE AGENCIES

Onward referral to Outside agencies in the community can be made following receipt of referral from your GP at any of the following points on the pathway.

  • Directly following triage
  • Following any appropriate assessment and or treatment
  • Following any investigations

At the point of onward referral the practitioner will discuss the specialist service that they feel is the most appropriate to continue with your care.

You will be supported to arrange an appointment to see these teams.

  • Move to improve
  • Self-management and support groups
  • Weight management
